In order to receive any additional documentation from the Esthetic Institute, other than those provided to you upon graduation, please fill out the form below.  

Please note that Student Records are only kept for 6 years. If your request dates fall outside of that period, we WILL NOT be able to assist you. 

Please note that there is a Fee associated with each type of request. (see list below) 

Once we receive your request, you will receive an invoice via email with the required fees.  

Once the payment has been received, it will take 5-7 business days for us to mail the document to your mailing address below or in case of official transcripts to the Institution you designate below.
